Nomination Deadline Extended to December 31st!

To make a nomination for a 2013 U.S. Water Prize, please download one nomination form from the list below, complete the form, and attach any supplemental materials to the document. If you are submitting the nomination form via email, please ensure the nomination form and supplemental materials are in one document. Letters of recommendation can be emailed/mailed separately.

Non-Individual Nomination

If you are nominating a non-profit, educational institution, company, government agency/coalition, utility, law firm or any other type of organization that is not a person, please use the nomination form below. This form may be used to nominate an organization that you work for or any organization you believe deserves recognition.
